Introduction
to Breast Augmentation Surgery
There are a number of different types of breast augmentation surgery,
enlargement using implants, reduction, lift, reconstruction, restructuring
and reshaping, partial surgery where one nipple may be slightly
higher than the other for example, and reconstructive surgery following
an accident or surgery.
The ideal solution may be a combination of surgeries, for example a breast reduction may
need a lift and also benefit from implants. Where possible we incorporate all surgical costs
within the prices. For example a breast reduction is charged as one price even though it
may also incorporate a lift. However where implants are required and not foreseen they may
be an additional charge. The ideal solution may be decided on during consultation where
the surgeon can advise on the different possible solutions and outcomes and assist in determining
the level of surgery required.
Breast Enlargement
An ideal solution for increasing the bust by one or more bra cup
sizes.
Our partner clinic will fit only cohesive gel implants; salt-water
implants are not sufficiently reliable. The implant is fitted through
an opening underneath the breast; any slight scarring will not be
noticeable.
What type
of Implants?
The cosmetic surgeon can fit either round or anatomical shaped implants
depending on the requirement.. Implants come in many sizes and can
even be custom made should the client require very large implants..
The latest silicones used in breast implants have high cohesion
which means that they are less liquid and therefore will not leak.
This greatly increases the durability of the implant even if the
implant receives a tear from an enormous impact (such as a car accident).
Because the latest silicones are firmer and keep their shape, they
are ideal for tear shaped implants often used in reconstructive
breast surgery. Implants retain their natural feel in spite of the
increased cohesion of the newer silicones. Nearly all implants have
a textured surface, allowing the implant to sustain a correct position
and not shift and turn, this also minimises sagging over time. Some
salt water filled implants specifically designed for placing underneath
the breast muscle do not have a surface.
High Cohesive silicone gel implants carry a lifetime warranty.
Anatomical
or Teardrop Shaped Implant
Teardrop shape anatomical implants create a natural shape and effect.
They are ideal for those with a relatively thin skin layer and little
breast tissue, however round or tear shaped implants produce beautiful
results for those with thicker skin.
With slightly sagging breast the mass of breast tissue is located
at the bottom of the breast.
Under these circumstances a round implant is more suitable as tear
shaped implants have most of their mass in the lower part of the
implant.
During
your consultation, the surgeon will take some pictures and measurements,
he will show you a number of pictures and explain carefully what
realitic results to expect so you can best choose the look and shape
that you desire.
The quality of the finished job is as important to the surgeon as
it to you.
Implants can be placed above, beneath or between the breast muscles.
Where they are placed depends on a number of factors including age,
weight, physical shape, breast shape.
Fixation
Over Muscle
The most common and natural way is to insert the implant above the
muscle, just under the breast enabling the implant to follow breast
movement for natural effect. However patients with thin skin and
minimal breast tissue may find that the edges of implants can be
felt at the top edge of the breast. With a liquid implants fitted
this way small ripples can sometimes be visible at the upper part
of the breast. When placed under the muscle, the chances of being
able to feel implant edges are greatly reduced.

Implants fixed under muscle are more fixed in position and so move
less easily and do not follow the natural breast movement as well.
When lying down the breasts tend to be upright. The cleavage between
the breasts becomes less apparent because implants cannot be placed
too close together.
Smooth, salt water filled implants are used when placing under the
muscle to make it feel more natural. When implants are larger than
300cc they are best placed under the muscle to prevent them from
drifting out of place and reduce sagging.

A recent development is a combination of the two techniques where
the muscles are separated and the implant is placed between the
muscles so that it becomes partly covered.
The upper part of the implant is covered so that the edges of the
implant cannot be felt and the implant is able to follow the breast’s
movement.
Breast
Sizes
Breast sizes are measured by weight in grams and are not referred
to as A, B or C cups. This is because cup sizes as used by bra manufacturers
vary. Generally it is agreed that 200gr is one-cup size. Breast
enlargements are the ideal solution for increasing a woman's bust
line by one or more bra cup sizes such as in women who have smaller
breasts.

Implants Carry a Lifetime
Warranty!
During your initial consultation, the surgeon will discuss with
you your expectations as well as the surgical possibilities.
Certification and registration of the implants lifetime warranty
is presented to the patient when they are treated.
Recovery is rapid and most daily activities can
be resumed after one week with your doctor's permission.
Breast Reduction
This is a safe and permanent solution for those
who are experiencing enlarged or sagging breasts. Can leave noticeable,
permanent scars. Your bra or bathing suit will cover these. (Poor
healing and wider scars are more common in smokers.) Future breast-feeding
may not be possible, since the surgery removes many of the milk
ducts leading to the nipples. Some patients develop small sores
around their nipples after surgery; these can be treated with antibiotic
creams. Following breast reduction some patients may experience
a permanent loss of feeling in their nipples or breasts.

Incisions outline the area of skin, breast
tissue, and fat to be removed and the new position for the
nipple. |

Skin formerly located above the nipple is
brought down and together to reshape the breast. Sutures close
the incisions, giving the breast it's new contour. |

Scars around the areola, below it, and in
the crease under the breast are permanent, but can be easily
concealed by clothing. |

Reduction with Breast Lift
Following weight reduction it is common to need a breast reduction
and a breast lift. Breasts are particularlyheavy and bearing
large breasts for a time will cause them to sag therefore
to get a good finished result a lift often accompanies a reduction.
Breast
reduction is called Reduction Mammaplasty, breast lift is
called Mastopexy.
The shape of the breast is corrected and the nipple is moved
to a more youthful or higher position, excess skin and breast
tissue are removed. The surgery takes place under general
anaesthesia and requires one night in hospital. It is not
a painful procedure and recovery is swift. The candidate can
resume normal activities within a week.

Contouring & Reconstruction
Breast contouring is reshaping the breast to reduce sagging
and malformed or misshapen nipples. Breast reconstruction
is often undertaken following breast removal or trauma associated
with accidents or burns. The breast nipple can be repaired
replaced or relocated.
Breast
Up-Lift
For sagging breasts and / or nipples that are positioned too
low, a breast uplift is sometimes necessary. With this procedure,
the nipple height is corrected, which results in a little
scaring around the nipple. Excess skin under the nipple is
removed.
If the nipple position is not too low breast
implants may be fitted to solve the problem. The more the
breast is sagging, the larger the prosthesis needs to be.
With medium sagging an augmentation by 1-2 cups is required
to return the breasts to natural firmness. This can be achieved
with limited scarring. Feeling in the nipples is still present,
so breast-feeding is possible. There are a number of different
breast lift possibilities.

Mini
Breast Lift (can lift the nipple by 4cm)
If the nipple position is slightly lower than it should be,
a mini breast lift could be performed. When measuring from
the depression where the collarbones meet, diagonally to the
nipples, the measurement should be between 22-25cm. With this
procedure only a small amount of scarring around the nipple
will be visible. The feeling in the nipples is retained and
breast-feeding is possible. By using a new type of stitch
thread, the surgeon limits the amount of scarring. The stitching
will be placed 1 cm away from the nipple. The mini breast
lift can often be combined with a breast implant. |

Internal Muscle Bra Fixation
(can lift up to 10cm)
This new technique is ideal for large sagging breasts.
By tightening a piece of the breast muscle, the breasts are lifted.
This technique is extremely successful and results are permanent.
Often, breast volume seems bigger after this procedure because the
breast glands are both lifted and tightened.
With exceptionally large breast volume, part of the lower breast
is removed, and the breast is rebuilt to give it new and younger
look.
Small Sagging = 22-25cm* and nipples facing down
Medium Sagging = 20-25 cm*
Large Sagging = 25 - 40 cm*
